The fall Season is here and Heritage Sierra Medical Group is here for you! We want to make it easy for you to make Heritage Sierra Medical Group your providers of choice(HSMG). Call our Member Services Direct Lines for Assistance: 661-579-2049 or 661-579-2056 Monday through Friday 9 a.m. to 4:30 p.m.
This is a special time of year called “Open Enrollment” that starts October 7, and goes up to December 15.  Three (3) months before individuals turn 65 and three months after they turn 65, individuals can apply for Medicare or a Medicare Advanatage plan and select Medicare physicians and specialty providers for comprehensive medical services and treatment.  If you already have Medicare or Medicare Advantage coverage, you can make changes to your coverage and select Heritage Sierra Medical Group {HSMG} as your new doctors.
HSMG’s Member Services representatives are available Monday through Friday, 9 a.m. to 4:30 p.m. to take your call so you can learn all about how to enroll in Medicare or a Medicare Advantage plan and select HSMG as your doctors.  You can also attend a Medicare enrollment event in your community.  Look for the Heritage Sierra Medical Group’s flyers, advertisements, Facebook and Twitter announcements and see special enrollment events advertising in your local newspapers don’t forget to check Facebook and Twitter.
HSMG has offices in Lancaster and Palmdale with Urgent Care services at each locations to take care of infants and seniors and every age in between of your entire family.
